There are 3 required steps you must take in order to register to attend the Healthy Community Institute (HCI).

1. Online registration form
• Team Members Attending: In registering for the Healthy Community Institute, you are committing your team to making a change in how your organization is currently operating in order to improve the agency’s effectiveness. At least three members of your team must attend, including the Executive Director and at least one board member. Registration will not be accepted unless this minimum requirement is met. A team may not exceed four members.
2. Organizational pre-assessment survey
• Complete this online survey to tell us a bit more about your organization prior to attending the Healthy Community Institute. This survey is the second step in the registration process and is required in order to confirm your participation in HCI.
3. $150 deposit
• Check made payable to "BCBSNC Foundation." (Note: the check simply holds your space and will be returned to you after your team completes the Institute.)
• Mail your check to the following address:

Keep in mind that there are a limited number of spots available. Registration may close early and is on a first-come, first-serve basis. If we have not received all 3 parts of your registration, including the check, you will not be registered to attend.

LOGISTICS

HCI is scheduled for October 11-12 at the Embassy Suites in Concord, NC. The Institute is two full days. 

A special rate of $129 per night plus tax at the Embassy Suites Hotel Resort and Conference Center is available for a limited number of rooms for participants. The rate is for single or double occupancy, third or fourth person in the room is $10 extra. You can book in one of two ways:

• Call the hotel directly at 704.455.8200 for overnight reservations. Ask for group reservations and reference the Healthy Community Institute when making reservations.
• Book online by visiting the Embassy Suites website and clicking "Book Now." After entering your information, enter the "Event Code" HCI.
