Examples of programs that would meet one or more of the results sought by this focus area:

• Creation of a centralized electronic patient records system that improves medical management of a vulnerable population
• Collaboration of a network of safety net organizations serving a specified financially needy, uninsured population (at no cost or on a sliding fee scale) in a particular county or region
• Provision of a chronic disease education and self-management program, targeting minority patients, that reduces the onset of and/or acuity of care of individuals with chronic disease
• Establishment of interpretation services for non-English speaking patients in order to reduce non-compliance with medication instructions resulting from low health literacy
