Strengthen Medicaid Transformation efforts in North Carolina – with a specific focus on facilitating cross-sector collaboration between health care and community-based organizations to address social needs – by delivering technical assistance, fostering a network of shared learning, capturing experiences, developing tools and resources for equitable and sustainable partnerships, convening across sectors, and acting as a resource hub.
